How is it done?
• Doctor requests the service.
• Home Health Care team will perform assessment.
• Equipment and supplies will be provided.
• Training and education will be done.
• The patient will be discharged from the hospital.
• The home health care staff will contact the patient to schedule frequent home visits until service is no longer required.
• Visits will be done between 08:30am to 03:00pm.
In case of any query, patients will be given a number to contact 24-hours a day, 7 days a week for special needs.
