Home Health Care Services
Home Respiratory Care Services for mechanically ventilated patient started in March 2009 with the success of this program, Home Health Care services was established in November 2009 to provide holistic service at patient's residence.
What is Home Health Care?
Home health care is a service that allows patients to receive medical care at home rather than staying in hospital. Home health care will provide medical team to visit you on a regular basis. Your equipment and medical supplies will be provided to you by home care.

To be under Home Health Care you need
- The patient must be referred by the treating physician from one of the components of the Riyadh Second Health Cluster.
- The requested service must be available through home healthcare.
- The home healthcare team must agree to treat the patient at home in accordance with admission policies.
- A caregiver must be present with the patient at home and have a means of communication.
- The patient or their home caregiver must sign a consent form to accept home care.
How is it done?
- Your physician requests the service through electronic referral.
- The home care team assesses the patient's health condition.
- Training and educating the patient and their family members.
- Providing medical equipment and necessary consumables.
- Scheduling home visits on an ongoing basis until the service is discontinued if no longer needed.
- The home care team visits occur during regular working hours.
- You are provided with a contact number in case of any inquiries or emergencies.

Patient's/Caregiver's Rights & Responsibilities
Patient's/Caregiver's Rights:
1. Home care patients/caregivers have a right to be informed verbally and in writing of their rights and obligations before they are enrolled under home health care program. The patient's family or guardian may exercise the patient's rights when the patient has been judged incompetent.
2. All patient shall be informed of the names of the doctors, nurses, and other health care professionals directly involved in their care.
3. Home care providers must protect and promote the rights of their patients, including ensuring they:
- Receive services without regard to race, creed, color, gender, age, disabilities, or lifestyle.
- Receive care of the highest quality in a safe and timely manner.
- Have relationships with home care providers that are based on honesty and ethical standards of conduct.
- Have their property treated with respect.
- Have the right to file complaints/ grievances to home care administration regarding treatment/care/service without fear of discrimination and reprisal of doing so.
- Are advised of the telephone number and hours of operation of the home health on-call line, land line and hospital for patient needs.
- Are informed of decisions about their care.
- Are aware that they may make their own decisions with regard to health care services.
- Are advised of any change in the plan of care before the change is made.
- Understand that they can refuse service/treatment.
- Are assured of confidentiality with regard to information about their health, social, and financial circumstances, and what takes place in the home.
- Understand that the home care provider will only release information regarding their diagnosis &/or treatment as required by law or as authorized by the patient.
- Will be told what to do in the case of an emergency.
Patient's/Caregiver's Responsibilities:
The patient/caregiver must be informed about his/her responsibilities which include the following:
- Not to video tape or audio record the home care team during the home visit to avoid legal accountability.
- Participating in goal setting/discharge planning that will impact positive outcomes during care.
- Compliance with medical advice/orders by active participation from patient and/or caregiver.
- Reading and understanding written information given upon admission to home care.
- Seeking clarification or further information when needed.
- Providing accurate and complete information to staff to ensure appropriate planning for care.
- Be available at scheduled home visit.
- Notifying home health care during the confirmation call if patient will not be home for the scheduled visit.
- Treating home care staff with respect and communicating problems that arise to appropriate staff immediately.
- Providing a female to accommodate the visiting team if the visiting team member is a female and a male when the visiting team member is a male.
- Commit proper use of equipment provided and obligated to return the medical equipment provided by Home Health Care if the patient is not using the equipment or if the patient passed away.
- Keep all pets away during the home visit.
- Providing a competent caregiver, could be trained by health care staff to perform daily tasks
- Provide an alternative competent caregiver in case the primary caregiver is not competent.
- Informing home health care of any changes in telephone numbers and/or address and accepting cancellation of the service if the new location is outside the geographical coverage of home care.
2. Understanding the Home Care services shall be withdrawn if any of the following occurs:
- Patient/caregiver refuses or rejects visits three times after confirmation without any valid reason.
- Patient not contactable for scheduling of visit three times in a row without valid reason.
- Patient/caregiver not compliant with advice regarding patient care which may be detrimental to patient's well-being.
- Patient/caregiver not compliant with proper use of any medical equipment issued.
- Patient/caregiver behaving inappropriately towards home health care providers.
- The patient has met his/her treatment goals.
- Patient medically stable more than 60 days.
- Duplication of home care service where patient is concurrently being followed up by other home health care services providers for same condition referred.
- Patient is readmitted to hospital or moved outside Riyadh for 60 days and more.
